Passed peacefully away on Ward C7, Russells Hall Hospital, on Friday, October 7, 2016, aged 64 years.

Much loved and sadly missed by all his Family and Friends.

Funeral Service to be held at Gornal Wood Crematorium, on Thursday, October 27, at 11am.

Family flowers only by request.
