Passed away peacefully after a short illness at City General Hospital, Stoke-on-Trent, on May 29, 2010, aged 63 years.
Funeral Service 12.30pm, Friday, June 25, at Telford Crematorium.
No flowers by request but if desired donations for The British Heart Foundation would be appreciated.
