Of Lancaster Avenue, Dawley, passed away at The Princess Royal Hospital, on April 18, 2010, aged 52 years.
Much loved Dad of Gemma and David, loving Grandad of Jayden and a dear Brother.
Funeral Service 12.45pm, Monday, April 26, at Holy Trinity Church, Dawley, followed by Cremation at Telford Crematorium.
Flowers welcome or if desired donations for The CCU at The Princess Royal Hospital, would be appreciated.
